- 12 to 17 years of age (upon admission)
- IQ greater than 69

-          Recommended contents of referral packets:  Psychiatric and Psychological Evaluations, Social History, Individual Education Plan (if Special Education), current Medical History and Physical, Previous treatment provider’s Progress Notes, including Medication History, Educational Information, and Discharge Packets from previous placements.
